Female pattern baldness (FPB), also known as female pattern hair loss (FPHL) or androgenetic alopecia in women, is a common condition characterised by gradual thinning of hair primarily over the crown and mid-scalp, often with a widening part line. While men tend to show receding hairlines, in women the thinning is more diffuse. The causes are multifactorial — genetics, hormonal changes (e.g., after menopause), nutritional deficiencies, stress, and imbalances in body systems (as per Ayurveda, in the doshas, rasa, rakta, meda etc.).

Ayurvedic Understanding of Female Pattern Baldness


Dosha imbalance: Usually Pitta (heat, inflammation) and Vata (dryness, degeneration) are involved. Excess Pitta can overheat scalp and damage follicles; excess Vata leads to dryness, weak hair roots, breakage. Kapha imbalance can lead to heaviness or clogged follicles.


Dhatu (tissue) depletion: Hair arises from the asthi (bone) and majja (marrow) dhatus, but is nourished by rasa (plasma/nutrition) and rakta (blood). Weakness or depletion in any of these tissues leads to thinning hair.


Ama (toxins) accumulation: Poor digestion (mandagni), exposure to environmental toxins, poor metabolic waste removal can lead to ama which blocks hair follicles and impairs nutrient delivery.


Hormonal and genetic factors: Androgens like DHT may contribute; menopause, thyroid disorders, PCOS, or other endocrine issues may have role.


Diagnostic Steps


Before treatment, it is crucial to assess:


Clinical history – onset, pattern of hair loss, family history, menstrual history, thyroid or endocrine disorders, stress levels, any scalp symptoms (itching, burning etc.), medications.


Physical examination – scalp condition, type of hair shaft, presence of dandruff or infection, signs of inflammation.


Laboratory tests – thyroid function, iron levels (ferritin), vitamin B12, zinc, hormonal profile if indicated.


Ayurvedic evaluation – assessment of prakriti (constitution), dosha imbalance, quality of agni (digestion), quality of dhatus, presence of ama, etc.


Ayurvedic Treatment Protocol


At Vedantam Ayurveda, we use a combined treatment protocol which includes external therapies, internal medicines, diet & lifestyle modifications, and supportive regimen. Here are the main components.


1. External Therapies (Samsodhana / Shamana)


Abhyanga & Shiroabhyanga: Regular warm oil massage of the scalp using medicated oils (e.g., bringharaj, amla, bhringamalakadi taila) to stimulate blood flow, calm Pitta, soothe Vata, nourish roots.


Shirodhara: Continuous pour of warm herbal oil or decoction over the forehead and scalp to reduce stress, calm the mind, reduce nervous tension, which in turn helps hair health.


Nasya: Use of medicated oils or herbal drops via nasal route; this influences the head region, improves circulation to scalp, calms Pitta.


Shirolepa / Lepa: Applying herbal pastes or masks (e.g., of amla, bhringraj, hibiscus, fenugreek, neem) to scalp to reduce inflammation, provide nutrients, cool the scalp.


Herbal washes & cleansers: Use of mild herbal cleansers such as shikakai, reetha, hibiscus, amla to cleanse without damaging the scalp or causing further dryness.


2. Internal Medicines & Herbs


Rasayana herbs for rejuvenation, e.g. Brahmi, Ashwagandha, Shatavari, Amalaki. These help modulate hormones, reduce oxidative damage, support resilience of follicles.


Triphala: Useful for cleansing, improving digestion and eliminating ama.


Lauha (iron) preparations, if iron deficiency is present.


Drugs or powders with Krimighna (anti-microbial), Asthi-Majjāpācaka properties to help digest bad tissue and support bone/marrow nourishment.


If hormonal imbalance is present, herbs which have mild anti-androgenic effect may be included (with supervision).


3. Diet & Nutrition


Balanced, nutritious diet: Rich in protein (legumes, nuts, dairy or substitutes), iron, zinc, essential fatty acids. Include amla, green leafy vegetables, fruits high in antioxidants.


Avoid overly heating, spicy or acidic foods when Pitta is aggravated.


Hydration: Adequate water intake; herbal teas (e.g. cooling herbs) to flush toxins.


Lifestyle: Good sleep, stress management (yoga, pranayama, meditation), avoid harsh chemical hair treatments or styling, avoid tight hairstyles that pull hair.


4. Panchakarma & Detoxification


Virechana (therapeutic purgation) in cases of Pitta excess.


Basti (medicated enemas) if Vata imbalance is significant.


Raktamokshana if indicated (bloodletting) in classical texts for certain scalp / blood disorders.


These purificatory treatments help remove deep-seated toxins and reset body systems.


Treatment Course & Timeline


Initial Phase (0-3 months): Focus on diagnostics, begin external therapies (oil massage, lepams), internal medicines, diet correction. Expect gradual reduction in shedding.


Consolidation Phase (3-6 months): Continue therapies; strengthen follicles; promote regrowth. Use more intensive treatments if needed (e.g. Shirodhara, Nasya).


Maintenance Phase (6-12 months and beyond): Prevent relapse. Maintain healthy lifestyle, periodic external therapies, internal rejuvenative herbs, and monitor any hormonal or nutritional deficits.


Results may not be immediate; hair cycles naturally follow phases; regrowth is slow. Patience and consistency are key.
